Online Form Request

NCBLPC will have new statutory requirements as of October 1, 2009.  At this time the Board  is in the process of updating all areas of the website to cover the new requirements.  Updated information, along with processes and forms, will be available for viewing as of October 5, 2009.  All applications received after close of business, September 30, 2009 will need to be on the new application forms and will be subject to all new requirements for licensure.  These requirements will also apply to all re-applications.

If you would like to request a new application packet prior to October 1, 2009, you may submit your request in writing to the Board’s office one of two ways:

1) To receive an electronic version via Email -    ncblpc@mgmt4u.com

2) To receive a paper version via US Mail - Submit written request along with the $5.00 application packet fee.  The Board’s address is listed below.
