your username and password you will need to direct your internet
browser to allow for pop ups from this site. The
Application Update Form needs to open in a new window.
This section is for Applicants to review remaining requirements
for licensure. Only Active Applications will be accessible. Once
an application has expired, been denied, or licensed, this information
will no longer be available.
A login and password will
be mailed to all applicants once their application has been posted
on-line. If you have misplaced this information, you may
